I-693 forms and immigration exams are completed by Dr. Paul Allegra, our Civil Surgeon at Advocate Urgent Care in West Saint Paul.

Please bring the following items to your appointment:

• a valid ID or passport

• vaccination records

  Pricing:

  • $500 for patients 15 years and older

  • $450 for patients 14 years and younger

  The fee includes the physical exam and required bloodwork.
